Camp Cullen Forms Due November 3

1. Complete the Google Form. Regardless of whether your child is attending or not, please complete the Google Form below.

2. Choose an option of payment
You must be logged in to Membership Toolkit to choose a payment.
There are many options: pay the fee, pay the fee in installments, donate for another student, or ask for a scholarship.
Money is not a factor in participating.

3. Complete 4 paper forms (packet sent home with student)

Camp Cullen Horse Waiver
Camp Cullen Challenge Course Waiver

4. If your child is taking any medications, even over-the-counter medicine, the medication form must be completed AND signed by the doctor.

5th Grade Promotion Fee

A $50 fee per student helps cover the 5th Grade t-shirt, end-of-year celebration, promotion ceremony, and other special 5th Grade events.

The 5th Grade Promotion Fee form is available under Account Information at conditpto.org. Please be sure to enter your child’s t-shirt size on the sales form.
