Payment/Reimbursement Request Form

If you need the PTO to pay a vendor or need to be reimbursed for a purchase, please complete this form and attach all receipts and/or invoices. Please allow up to two weeks for payments to be processed.

 

Condit PTO is a tax-exempt organization under section 501 (c)(3) of the Internal Revenue Code, and is therefore exempt from paying sales tax on purchases. As such, we do not reimburse individuals for sales tax.  To avoid paying sales tax, please use our tax-exempt form.

*PSI Volunteers Needed

The Preparing Students for Independence (PSI) class at Condit is a wonderful program that supports students who require specialized support. We are requesting volunteers to help escort PSI students to and from enrichment by assisting with strollers or wheelchairs alongside the teaching assistants.

Your support allows these students to enjoy enrichment classes, spend time outside their classroom, and ensures their teacher has proper planning time.

Details:

Time: 9:50 AM–10:30 AM
Commitment: At least one day (or more!) every other week during year
Requirement: VIPS approval

This is a meaningful way to give back and be part of our Condit community. Just 30–40 minutes every other week will make a big difference!
